We conducted an assessment, what's next? In June, the UNITE (Unified Needs Assessment and Implementation Plan Team Engagement) group at the WHI published an updated Community Health Needs Assessment (CHNA) for Washtenaw County. Follow the link below to review it. This fall, the group will also publish an implementation plan using findings from the assessment. The plan will guide how the hospitals work to address community needs.

Who isn't participating in Annual Wellness Visits, and why? Annual Wellness Visits (AWVs) are preventive care visits between Medicare beneficiaries and their primary care providers. While AWVs are beneficial for patients and health care providers alike, the use of AWVs is low nationwide, especially among individuals with low incomes and ethnoracial minorities. The WHI and its partners hope to identify who is and isn’t using Annual Wellness Visits in Washtenaw County and the root causes for any disparities they find. Then, they will work to build a strategy for increasing AWV participation.
