NEW CIE PARTNERS
CIE San Diego is excited to announce our newest CIE Member
Kaiser Permanente. 

More than 6,000 KP members have been served through the CIE over the years. This new partnership will enable us to more effectively coordinate care for them and those with future needs.  

Kaiser Permanente recognizes good health is more than what happens in the doctor’s office. It includes having a safe place to live, a steady supply of healthy food, social support, and access to other daily essentials. To address these patient needs, Kaiser Permanente supports networks of health and social service providers in every community it serves across the country.
 
Recognizing San Diego’s robust, community-developed care coordination network already in place -- the 211 San Diego Community Information Exchange (CIE) -- Kaiser Permanente, supported by their technology partner Unite Us, is implementing an integration with San Diego’s 211 CIE platform. Through this data integration, Unite Us is providing the referral tool technology for Kaiser Permanente staff to send secure electronic referrals to the 116 (and growing) CIE Network Partners via existing CIE technology and workflows.
 
The vision for the partnership is to increase the number of people connecting to San Diego’s diverse resources. Kaiser Permanente’s connection to the 211 San Diego CIE launched on May 2. It is initially being managed manually while we complete the electronic integration slated for late summer. The initial manual “work-around” and the final integration will look and feel the same to CIE partners. This partnership exemplifies our mutual commitment to open and collaborative networks.

NEW CIE DIRECT REFERRALS
Access, Inc

Offers educational and employment services for youth and provides quality individually focused access to workshops, hiring events, training programs, and other resources such as mentors, community supportive services, and leadership training.

Based on individual goals, interests, and circumstances, youth are supported in attaining high-school diplomas, enrolling in post-secondary education and/or occupational/technical skills training, and designing plans to prepare for their selected career pathways.

Services are provided at more than ten accessible locations throughout San Diego County as well as remotely.
Molina Healthcare

The Case Management Department offers a needs assessment and develops an individualized plan to address Molina Healthcare’s eligible members’ needs. The Case Management Service Team can assist participants in navigating the health care system, connection to available resources, community-Based Adult Services, Multipurpose Senior Services Program, transportation resources, and other social support services.

Serves Molina Health Plan members who could benefit from case management services.

Elder Law and Advocacy

The Health Insurance Counseling & Advocacy Program (HICAP) is California's state program for Medicare and Long-Term Care Insurance information. Elder Law and Advocacy HICAP counselors are registered with the California Department of Aging to help Medicare beneficiaries:

  • understand and compare their Medicare or Medicare-Medi-Cal coverage options, such as Medicare Advantage plans, Medigap supplement plans, Part D drug plans, and dual-eligible plan options
  • find out if they are eligible for low-income assistance and other strategies to reduce premiums, deductibles, copays, and drug costs 
  • resolve billing issues and get legal help with appeals
  • learn about long term care coverage options

Medicare beneficiaries can contact HICAP Monday through Friday, 8 am - 5 pm, to schedule an appointment with a HICAP Counselor or get help by phone or email.

SAN DIEGO GIVES
211 San Diego is partnered with San Diego Gives to offer San Diego nonprofits months of networking and training in fundraising, donor stewardship, and communications, culminating in a huge day of local giving on September 8, 2022. We encourage 501(c)(3) nonprofits serving the greater San Diego area to sign up for this year’s experience before May 30, 2022. We’re excited to help donors “give local” and recognize the crucial work of our nonprofit partners on September 8, 2022!
COVID-19 At Home Test Kits
The County of San Diego Health and Human Services Agency HHSA has received COVID-19 self-test kits from the state. Distribution criteria and a distribution plan have been developed to ensure that those at highest risk of illness and death from COVID-19 infection and those in low income and underserved communities receive these kits. 
Kits will be distributed based on the following criteria: 


  1. Community-Based Organizations (CBOs) that are located in the health equity zip codes. The health equity metric measures the positivity rate in the most disproportionately impacted communities. These communities are identified in the healthy places index as census tracts that have less healthy community conditions such as low median income, education completeness, and health care access. 
  2. Agencies that provide services to individuals and families that earn an income of less than or equal to 200% of the Federal Income Guidelines (i.e., Medi-Cal, and State and Federally funded Head Starts and State Preschools).
  3. Populations that have lower vaccination rates and higher rates of disease and deaths from COVID-19 (i.e., African American/Black, Hispanic, and Native Hawaiian/Pacific Islander).
  4. Rural areas with limited COVID-19 testing sites (i.e., Borrego Springs, Julian, Pine Valley, Potrero, Ramona, Valley Center).

Aligning California: Maximizing Opportunities to Advance Local Community-Led Networks

California is leading the nation in community health and systems change innovation. Recent local and state policy proposals prioritize funding to support data-sharing infrastructure, with an emphasis on technology-enabled person-centered care models. The Community Information Exchange (CIE) model can be used to design systems that meet the needs of Medicaid recipients and other vulnerable populations while prioritizing local community governance.

We’re bringing together CIE organizations and other stakeholders from across California to explore how we can leverage the CIE to achieve systems change and prepare for future funding opportunities that support community health.

Affinity Group Sign-Up
 CIE San Diego wrapped up the final session of their first pilot affinity group session on May 2! 

A special "Thank You" goes out to Access Inc., Global Communities, Oceanside City Library, and Say San Diego for their participation. 

What is an Affinity Group? It is a "learn & work" space where members from different organizations develop their CIE implementation plan amongst peers.  

This is a supportive environment for partners to brainstorm, problem-solve and innovate.  
  
 The goal is to define use cases, outline processes, and strengthen partnerships using CIE.

CIE Partner Gathering

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.

Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via zoom.
