Association of Patient and System-Level Factors in Social Determinants of Health Screening

This presentation covers the quantitative findings from an enterprise-wide implementation of an EHR-based social determinants of health screening tool at the Mayo Clinic that took place in June 2019. The results focus on the association of patient and system-level factors with completion and modality of screening and how completion changed over time. The speakers will provide a contextual overview of continuous efforts at Mayo Clinic to improve response rates and support provider identification and treatment of Social Determinants of Health risk factors.
