Team-Based Care for Essex County Seniors Aims to Promote Independence, Reduce Hospitalization
Zufall Health has established a new model of care for vulnerable Essex County Seniors, thanks to a generous award from the Millburn-based grantmaking organization the Healthcare Foundation of New Jersey. Patients 65 and over in the county will enjoy team-based care from a geriatric nurse practitioner, nurse case manager, clinical pharmacist, and outreach worker. The program aims to help our seniors remain independent and in their communities, while limiting the need for hospitalization.

Welcome Our New Chief Operations Officer (COO), Annette Gonzalez, DHA, MPA
Annette Gonzalez, DHA, MPA, a health care executive with more than 30 years of experience, was appointed COO in May. Gonzalez replaced former COO Frances L. Palm, MPA, who became president and chief executive officer in February. Her past experience includes serving as director of ambulatory care for NYC Health + Hospitals Lincoln in the Bronx, where, like Zufall, many patients identify as Latinx/Hispanic, are best served in a language other than English, and/or are uninsured.

Anxiety and Depression Are Common for New Moms; Spanish-Speaking Zufall Support Group is Here to Help
Pregnancy and early motherhood can be difficult for anyone, especially those who face linguistic and cultural barriers to care. Licensed professional counselor Nereida (Nettie) Mendez, LPC, NCC, and colleagues in Zufall’s behavioral health department are providing free support to Spanish-speaking mothers on Zoom in a new support group called “Esperanza Para Las Madres” (Hope for Mothers). The group was formed in collaboration with the Partnership for Maternal and Child Health of Northern New Jersey.
