Team-Based Care for Essex County Seniors Aims to Promote Independence, Reduce Hospitalization

Patients 65 and over in the county will enjoy team-based care from a geriatric nurse practitioner, nurse case manager, clinical pharmacist, and outreach worker. The program aims to help our seniors remain independent and in their communities, while limiting the need for hospitalization.

What We Can Learn from a 1991 Letter from Our Founder

Robert Zufall, M.D. (“Dr. Bob”) recently celebrated his 99th birthday, and so we are sharing pieces of his history during his 100th year of life. In 1991, shortly after he and his wife founded the Dover Community Clinic—today known as Zufall Health—he wrote about our health center in the Journal of the American Medical Association, a highly respected medical publication.
