The National RAISE Center will be hosting its 10th Summit in St. Louis, Missouri. It will be a time for leadership development, networking and team building. We have enabled a virtual option, so if you are not attending in person, you can still participate virtually.


Date: Thursday, July 25th

Time: 9am - 4pm Central Time (10am-5pm ET)


This summit will focus on developing and strengthening relationships with key partners to improve transition outcomes.

9 AM-9:15 AM- Welcome


9:15 AM-10 AM- A Symphony of Community Voices: MPACT’s Regional Transition Network (RTN) Model - MPACT


For more than ten years, MPACT has facilitated youth-focused interagency collaboration groups, called Regional Transition Networks (RTNs), to improve postsecondary outcomes among youth with disability labels. This presentation will describe key components of RTNs, including their varied membership, resource-mapping and action-planning procedures, and community transition events that network members plan and put on. We will also share benefits and barriers of this model, along with feedback from youth, family, and professional participants. 


10 AM – 10:45 AM- Exploring the Intersection: Parent Centers and Vocational Rehabilitation Services - A Deep Dive – Susan Barlow


Explore the latest insights gleaned from our VR Survey. Join us for a dynamic session where we not only unveil the findings but also delve into effective strategies, fostering a collaborative exchange where states can showcase successes and exchange ideas to enhance their foothold in the immersive VR landscape.

11 AM- 11:45- LifeCourse in Health Care Transition - UMKC


This session will explore the innovative LifeCourse approach to health, emphasizing the importance of healthy living across various life domains and the critical transition to adult healthcare. Drawing from resources like LifeCourseTools.com and MovingToAdultHealthcare.org, we'll discuss strategies for promoting wellness throughout different life stages, ensuring a holistic understanding of health that integrates physical, emotional, and social well-being.


11:45 AM-12 PM- Break and Grab Box Lunch


12 PM-1:15 PM- RSA PC Spotlight


Discover innovative resources from Regional RSA Parent Centers aimed at supporting families and youth in transition, with each center presenting an online tool to enhance support networks. Join us to explore these valuable tools and strategies for smoother transitions.


1:45 PM-3:30 PM- Youth Engagement Now


This session will showcase the youth led, youth driven "Youth Engagement Now" Project. Youth leaders from across the U.S. joined forces to develop a set of tools for individuals or organizations to effectively engage youth and young adults with disabilities. Join us to hear how the project came together and from the young adult leaders who developed the YEN Project. Attendees will gain access to the tools and discuss ways to use them.


3:30 PM-4 PM- Wrap up, takeaways, plan ahead, evaluations.
