Additional Plan Information Online

If it’s been a while since you last looked at your plan information, you and your employees should check out our newly updated Member Resources page on our website.


This page now features links to detailed information about all of Allied National’s health plans.


To find the name of your plan, check your ID card. You’ll be reminded of the special benefits of your plan and who to contact if you need more information.


However, to find even more detailed information, you can read your Summary Plan Description (SPD), which is located in your Self-Service Site at www.alliednational.com.

Freedom Hybrid Resources

Elite Experience Team

Do you or your employees have questions about your Freedom Health Plan? Are your providers asking questions about your non-PPO health plan you can’t answer? Allied’s Elite Experience Team is here to help our members and providers navigate their Reference-Based Pricing health plan.


Members should contact the Elite Experience team if they have a provider that doesn’t recognize the non-PPO Freedom health plan, which allows members to see any provider without penalty. The Elite Experience Team will work with the provider to explain the benefits and all aspects of your Freedom Health Plan. As Freedom Plan members you are responsible only for copays, deductibles and coinsurance as shown in the Explanation of Benefits.


If a member receives a balance bill for anything other than their copays, deductibles and coinsurance they should immediately call Allied’s Elite Experience team at 866-332-1987 or email a copy of the bill to elite@alliednational.com. Members are not responsible for any balance billing from providers who won’t accept the reimbursement levels of the plan.


Through our Elite Experience Team, Allied provides a legal support service for members who are being subjected to balance bill collections. The Legal Support service removes the member from the balance bill appeals process.

5500 Filing and PCORI Fee Due July 31

The deadline for calendar-year plans to file Form 5500 electronically with the Internal Revenue Service (IRS) is Monday, July 31, 2023. 


Payment of PCORI fees also is due July 31. For plan years that ended on or after Oct. 1, 2022, and before Oct. 1, 2023 (including calendar year plans), the fee is $3 per person covered by the plan. For plan years that ended after Oct. 1, 2021, and before Oct. 1, 2022, the fee is $2.79 per person.
