As you know this survey seeks to differentiate high supply chain risks from low supply chain risks for the 4th Quarter of 2022.

This survey will take less than 10 minutes to finish where you will be asked to look at 10 supply chain risks and select whether the risks will increase, remain the same or decrease in the next quarter. The responses and your identity will remain anonymous. Please complete the survey next week if possible.
